Generally, when you write notes for a day or after reading something you are writing transient notes (Most people take only transient notes).
You never really re-visit these notes or need to rely on their quality in the future.
The quality of these notes are probably weaker because you do not iterate on them.
You need other processes that cause you to revisit old notes if you want to improve their quality.
In general, people don’t have a clear picture of what a note should be, so it’s not clear when a given note fails to conform to that standard.
